Why FAPI PET Outperforms CT in Ovarian Cancer Staging

Why FAPI PET Outperforms CT in Ovarian Cancer Staging
Accurate preoperative staging using FAPI PET ovarian cancer imaging can fundamentally change surgical selection. Recently, a prospective study from Bengaluru evaluated 200 patients with primary epithelial ovarian cancer. The researchers compared gallium-68 FAPI-04 PET/CT against conventional contrast-enhanced CT scans before cytoreductive surgery. Furthermore, surgeons used intraoperative exploration as the reference standard to confirm disease extent. The investigators published their landmark findings in the Journal of Nuclear Medicine.
Overcoming Limitations of Conventional CT Imaging
Peritoneal carcinomatosis occurs commonly in advanced ovarian cancer. Consequently, precise preoperative mapping dictates whether patients undergo primary surgery or neoadjuvant chemotherapy. However, conventional contrast CT frequently underestimates peritoneal deposits. Small, flat implants along the bowel serosa and diaphragm remain notoriously difficult to resolve. In contrast, FAPI PET targets fibroblast activation protein expressed on cancer-associated stroma. As a result, the tracer displays very low physiological uptake in normal bowel, yielding remarkable clarity.
Clinical Benefits of FAPI PET Ovarian Cancer Imaging
In this study, FAPI PET achieved a remarkable diagnostic sensitivity of 81.8% for peritoneal disease. In comparison, conventional CT reached a sensitivity of only 54.6%. Moreover, the overall accuracy reached 86% for FAPI PET versus 78% for CT. CT also consistently underestimated disease burden. Specifically, the mean Peritoneal Cancer Index was 15.7 at surgery, 14.1 on FAPI PET, and merely 10.8 on CT. Therefore, FAPI PET aligned much more closely with true intraoperative tumor burden.
Impact on Surgical Decisions and Future Adoption
Dr. Somashekhar SP noted that accurate mapping prevents futile exploratory laparotomies. Importantly, the scan findings altered therapeutic decisions in roughly 18% to 21% of surgical candidates. Additionally, clinicians may avoid invasive diagnostic laparoscopies in select patients. Currently, this imaging technology remains available at a limited number of Indian cancer centres. Nevertheless, wider clinical implementation could soon establish new standards for gynecological oncology staging across India.
Frequently Asked Questions
Q1: Why is FAPI PET superior to FDG PET in peritoneal disease?
FDG PET relies on glucose uptake, which often produces high physiological bowel activity. In contrast, FAPI binds to tumor stroma, offering low background signal and sharp lesion conspicuity.
Q2: How did FAPI PET influence treatment pathways in the study?
Specifically, scan findings led clinicians to modify therapeutic management in 18% to 21% of patients. Consequently, doctors could better tailor surgery and neoadjuvant therapy.
Q3: Is FAPI PET widely accessible across India?
Currently, FAPI PET is available at only select tertiary oncology institutions in India. However, clinicians expect availability to increase as prospective evidence expands.
